You may call us during our regular office hours for information about care that we have provided or questions about our services. If you have a question about care, it is always best to contact your child's doctor before calling us.

You may download our standard form as a pdf file. Please fill in your child's name, initial and sign before arriving at the office. We will then only need to copy your driver's license and your child's insurance card before starting to treat her or him.
